Physiotherapy Management of Scoliosis Using Infrared, TENS, and Klapp Exercise: A Case Report

Abstrak

Background: Scoliosis may be accompanied by pain, restricted spinal mobility, and impaired function. Evidence on the combined use of infrared (IR), transcutaneous electrical nerve stimulation (TENS), and Klapp Exercise in an individual case remains limited.
Objective: To describe changes in clinical outcomes following a four-session physiotherapy program combining IR, TENS, and Klapp Exercise in a patient with scoliosis. 
Methods: This case report involved a 12-year-old boy with scoliosis and a Cobb angle of 50°. He received four physiotherapy sessions at the Medical Rehabilitation Unit of Muhammadiyah Lamongan Hospital. Outcomes were assessed from T1 to T4 using the Visual Analog Scale (VAS), trunk range of motion, a scoliometer, Cobb angle, leg length discrepancy (LLD), and the Scoliosis Research Society-22 (SRS-22). 
Results: Resting pain decreased from 2/10 to 0/10, tenderness from 7/10 to 5/10, and movement-related pain from 5/10 to 3/10. Trunk flexion increased from 65° to 85°, extension from 20° to 30°, and lateral flexion from 25° to 30°. The total SRS-22 score increased from 51 to 63. Scoliometer findings remained 5°, Cobb angle remained 50°, and LLD remained 1.5 cm. 
Conclusion: In this case, four sessions of combined IR, TENS, and Klapp Exercise were associated with improvement in pain, trunk mobility, and SRS-22 score, whereas structural measures did not change during the observed treatment period.
